COVID booster vaccination request

Use this service to request your COVID-19 booster vaccination.

You can use this service if you:

• are registered at the surgery
• have been invited to do so

COVID booster vaccines can be given 180 days after the second dose. Patients attending the surgery prior to this will not be able to have the booster vaccine and will be asked to re book.
