New Patient Registration
The doctors welcome new patients who live within our practice area.
Click on the map on the right then enter your postcode to see if you live within our catchment.
Just turn up at either surgery and ask for a New Patient Registration Form (GMS1). Once this has been completed, along with a practice questionnaire, you will be registered with the practice. Copies of the forms to complete are available on the 'Registering Online' tab. Please note that patients will be asked to bring in photo ID and proof of address in order to register.
Allocating A Doctor
You have a right to request registration with a named doctor if you wish. If not then the practice staff will register you with any of the doctors.
All new patients to either surgery are entitled to a 15-minute health check with a Health Care Assistant. This is important because it can take up to 6 weeks to receive your medical records from your previous doctor’s. Your blood pressure, urine, current medication and past medical history will all be checked at this screening.
Please note that although you will be registered at one of the medical centres you can be seen at either site.
